Nope... can't do it just in the Terrain editor. To make a cave, you'll need to make the cave an interior (.dif) using QuArK or WorldCraft. Then for the terrain brush setting, select "set empty" and paint over the blocks you want to disappear. Then place your .dif of the cave appropriately under the terrain. It takes a bit of practice to make the dif just right so you don't see any holes around the entrance. Good luck!

Milkshape has a simple MAP exporter now, and it supports limited HL entities(translucency,lights, breakables, player_start/spawn~more to follow...), I brought some shapes into VHE and it was simple, texture coordinates don't follow but defined textures are applied if found within a WAD loaded for the construction. The simple room provided as an example was not much different than the Beach House interior map "WATER WORLD". I think this plugin will be expanded in the future.
